You Need:
Graphic, or use one of your own
Simple Plugin

1. Open a new image 1024 X 350

2. Selections - Select All

3. Open your graphic and Copy

4. Go back to your background image, right-click - Paste Into Selection

5. Effects - Plugins - Simple - 4 Way Average - Ok

6. Effects - Plugins - Simple - Half Wrap - Ok

7. Selections - Select None

8. Select your Dropper Tool and choose a dark color from your image as your Foreground color

9. In your Materials box, have your Background set to Pattern and choose your current background image as your pattern
Angle: 0, Scale: 100

10. Select your Text Tool and add your text

11. Objects - Align - Center in Canvas

12. Effects - 3D Effects - Drop Shdadow
V: 5, H: 5, Opacity: 50, Blur: 5, Color: Black

13. Layers - Merge - Merge All (flateen)

14. Image - Add Border
Color: Black, Size: 10

15. Select your Magic Wand Tool and click on the black border

16. Effects - Reflection Effects - Kaleidoscope, use these settings:

17. Effects - 3D Effects - Inner Bevel, use these settings:

18. Repeat step 14 & 15

19. Repeat step 16, but with these settings:

20. Effects - Texture Effects - Texture, use these settings:

21. Repeat steps 14, 15, 16 & 17

22. Resize is needed

And your all done!
