You need:
Kiwi's Plugin
VM Natural Filter
Masks
Graphic of your own

1. Open a new image 1024 X 350

2. Selections - Select All

3. Open your graphic and Copy

4. Go back to the background image, righ-click Paste - Into Selection

5. Effects - Plugins - VM Natural - Splash:
130, 130, 75, 15, 25

6. Effects - Plugins - Kiwi's oplfilter - Guk mal
155, 0, 128, 128

7. If your masks are open; Layers - New Mask Layer - From Image
Choose the Serenade mask, Invert mask data UNCHECKED

8. Layers - Delete - Yes

9. Layers - Merge - Merge Group

10. Go to your graphic image, Layers - New Mask Layer - From image
Choose the 20-20 mask

11. Repeat steps 8 & 9

12. Copy and Paste in your background image as a new layer

13. Select your Move Tool and move your graphic if needed

14. Select your Text Tool and write your text if desired

15. Layers - Merge - Merge All (flatten)

And that's all to it!
